Job Description:
A company is looking for a Care Manager responsible for managing
at-risk patient populations and coordinating complex case
management.
Key Responsibilities
Conduct health risk assessments and develop individualized care
plans for complex case management patients
Coordinate care transitions and collaborate with healthcare
providers, patients, and caregivers to facilitate effective
discharge planning
Manage a caseload of complex and rising risk patients, ensuring
adherence to clinical criteria and documentation standards
Required Qualifications, Training, and Education
High School Diploma or GED equivalent
BSN degree from an accredited university
Minimum of 2 years of case management experience in a managed care
environment
Minimum of 1 year of complex case management experience
Registered Nurse state licensure and/or compact state licensure
required; CCM certification preferred
